2016 – 2017 CCHS PARKING PERMIT APPLICATION
In order to purchase a CCHS parking permit, you must attach the following information to this completed application.
Copy of Proof of Liability Insurance – STUDENT’S NAME & VEHICLE must appear on the insurance card
Copy of valid Driver’s License – Must have driver’s photo on license – NO LEARNERS PERMIT
$35.00 parking fee. Check or Money Order should be made out to CCHS (include parent phone number)
